Iran’s new Supreme Leader Mojtaba Khamenei is still recovering from severe facial and leg injuries suffered during the attack that killed his father on the opening day of the war.
Sources who spoke to Reuters said Khamenei’s face was disfigured and he has suffered significant damage to one or both of his legs.
The account tallies with a much earlier report by US Secretary of War Pete Hegseth, who said on 13 March that Khamenei was “wounded and likely disfigured.”

He is said, nevertheless, to be making a good recovery and “remains mentally sharp.” He is currently taking part in decision-making on important issues by video link, including the negotiations with Washington.
Reuters notes that “Khamenei’s whereabouts, condition and ability to rule still largely remain a mystery to the public, with no photo, video or audio recording of him published since the air attack and his subsequent appointment as his father’s replacement on March 8.”
It was rumoured he had been taken to Moscow for emergency treatment.
As well as his father, the previous Supreme Leader, Mojtaba’s wife, brother-in-law and sister-in-law were also killed in the first-day strike.
Experts say it is unlikely, given not only his wounds but also his inexperience, that he will ever wield the kind of power his father had.
“Mojtaba will be one voice but it will not be the decisive one,” an analyst told Reuters.
“He needs to prove himself as the credible, powerful, overriding voice. The regime as a whole has to make a decision in terms of where they are going to go.”
The news comes as US Vice President JD Vance arrived in Pakistan for peace talks, in the first official meeting since the war began.
He is joined by special envoy Steve Witkoff and President Trump’s son-in-law Jared Kushner.

Although Vance has said he believes the negotiations will be “positive,” the talks face significant barriers, as Israel and Hezbollah continues to trade attacks along the southern border of Lebanon.
Both delegations have arrived in Pakistan’s capital Islamabad.
The Iranian delegation is led by Parliament Speaker Bagher Qalibaf, who was said that discussions will only happen if Israel agrees to a ceasefire in Lebanon and releases blocked Iranian assets.
President Trump, meanwhile, has said Iran has “no cards” in the negotiations, and that it is using the Strait of Hormuz, which remains closed, to extort America and the rest of the world.
“The only reason they are alive today is to negotiate!” the President wrote on Truth Social.
According to Pakistan’s Prime Minister Shehbaz Sharif, who helped broker the talks, the war is at a “make-or-break” moment.
Negotiations between Israel and Lebanon are expected to take place on Tuesday in Washington DC.
The Lebanese government wants to bring an end to the war between Israel and Hezbollah, under a ceasefire similar to the one negotiated with Iran, but Israel is pressing for the Lebanese government to disarm Hezbollah fully—but it’s unclear whether this can be done.
When the current truce with Iran was announced, Israel hit Lebanon with massive strikes, killing 300 people in the deadliest day of attacks against the country since the war with Iran began on 28 February.
